Form 10: Internal Academic Approval of Sponsored Programs The Form 10 is the internal academic approval form for sponsored programs. The form should be completed and submitted with all proposals. The current version of the Form 10 is dated 04/29/04. If you are using a version with an older date, please download the form below. Note: Due to the complications with the multi-platforms of Word versions, we have decided to eliminate the Word version of the Form 10. We recommend using the PDF version for all users. Some units and sponsors may require an addendum to the Form 10 for proposals. Follow the instructions below. If you have any questions about the the requirements , contact the administrative staff for your college or unit.font> NIH The National Institutes of Health requires Cornell to maintain a Principal Investigator Assurance. This Form 10 addendum must be use for each NIH proposal or approval request. College of Arts and Sciences College of Engineering Proposals from the College of Engineering require a Form 10 addendum and must be routed to Engineering College Administration under the following circumstances; proposals totaling $1million or more and/or proposals with Engineering College or University cost sharing commitments (excluding the 50% graduate tuition). Cornell University Library All Cornell University Library proposals require the advance approval of: 1) department/unit head(s) and the Associate University Librarian(s) whose units are affected by the proposal and 2) the Director, Finance & Administration, for proposals impacting any endowed library units. Their signatures are required on the Form 10 Addendum if they are not already on the Form 10.
